Help With Full Screen Mod

I've searched for help on installing the full screen mod from FMods with no luck.
When I go to install the 4th file nothing happens. Would any one have what files and folders should be on that flash drive.

Open the ZIP-file, go into folder "dist". Copy the files in this folder to your USB-root.
You don't need the other files and folders in this ZIP-file.

your root should look like this:

Folder "SyncMyMod "
File "DONTINDX.MSA"

Nothing more, nothing less.

For some reason, the F-Mods team responsible for the 4th USB put the USB contents in a separate folder in their zip file instead of just unpacking the zip file to USB. You have to find the folder and place contents on the the USB drive. See Pat's response above.
